Cornea is the transparent, dome shaped front portion of the eye, covering iris, pupil and the anterior chamber. Cornea provides most of the optical power of the eyes, and thus helps the eye to focus on any object. It is a powerful refracting surface.

An adult cornea is 11.5 mm in diameter, has a 0.5 mm thickness and consists of five layers. The cornea does not have any blood vessels and gets oxygen direct from the air. The transparency of the cornea is due to the presence of collagen fibres, and anything that affects these collagen fibres will cause serious corneal problems like corneal dystrophy.

Cataract is an unhealthy medical condition that develops in the lens of the eye. It prevents light from entering the retina of the eyes. Cataract affects all age groups and in an advanced stage, the cloudiness of the lens increases resulting in a decrease in the light reaching the retina. This will lead to a significant sight loss, and eventually, total blindness. Diabetes, glaucoma, smoking, eye injuries, infection etc are also known to cause cataract.

In people with keratoconus, until a few years ago, we had no choice but to replace all three layers of the cornea to correct their problem. This was a good option, called a corneal transplant. It involved using a small round blade to cut out the central thin part of the cornea. Success rate has been excellent, with most people improving vision to 20/40 or better after surgery. Yet in keratoconus and other corneal problems, affecting the outer layers of the cornea, it was a shame to have to remove the normal, inner layer of the cornea. This thin layer works as the water pump, pumping out the excess fluid from the cornea. It is also the layer that is most likely to reject, if a transplant rejection should occur. So why not retain this layer? With recent, modern techniques this is now possible to do by splitting the cornea between the middle and inner layer. This can be done with a special instrument called a dissector, by using air or fluid to separate the layers, or by means of a laser. This preserves the integrity of the eye by only replacing the layers of the cornea that are affected most, being the middle layer. This is a big advance for people with keratoconus, giving vision equivalent to that obtained with an all-layer corneal transplant with a much reduced risk of rejection.

What is DSAEK? DSAEK is a form of corneal replacement that replaces only the inner layers of the cornea. To understand DSAEK one needs to understand the structure of the cornea. The cornea is the round, transparent dome on the front of the eye and covers the eye in much the same way that a watch glass covers the surface of a watch. It is curved and smooth. The cornea is made up of three layers, much like an Oreo cookie. There is an outer wafer, and thick inner filling and an inner wafer. A corneal transplant replaces some or all of the layers of the cornea. Until a few years ago, in fact, there was only one good choice and that was to remove and replace all layers of the cornea. This was done with a circular blade, like a tiny cookie cutter that would cut out a disc of the central cornea to be replaced by a donor.
However, people that require a corneal transplant may only have problems with one layer, commonly the inner layer or wafer. This can occur as a result of an aging process called Fuchs’ dystrophy, can result from an injury or following eye surgery. A regular corneal transplant works well for these conditions, but does have its problems. The cornea can be not smooth and irregular following a transplant, having what we call astigmatism. This can be caused by the tension on the stitches that hold the transplant in place. Also, a regular corneal transplant takes a long time to heal, a year or more. During this healing time, the vision can be poor.
